Sole proprietors and sole proprietorships are Entity Type 1 (Individual) providers. There are two categories of health care providers for NPI enumeration purposes.Įntity Type 1 providers are individual providers who render health care (e.g., physicians, dentists, nurses). Identification Number (UPIN), Online Survey Certification & Reporting (OSCAR) and National Supplier Clearinghouse (NSC) in HIPAA standard transactions. If using UPIN number, use the qualifier 1G followed by the 6-digit number. If using the NPI Number, indicate this by using the qualifier XX followed by the 10-digit number. The NPI must be used in place of legacy provider identifiers, such as a Unique Provider PHYSICIAN Accurately indicate the treating physician’s Unique Physician Identification Number (UPIN) or applicable National INFORMATION: Provider Identifier (NPI). The NPI is introduced to replace of UPIN (unique provider identification number) and now NPI is the only required identifier for Medicare services, and NPI is also used by commercial healthcare insurers and by other payers. National Provider Identifier (NPI) is a 10-digit identification number which is issued to health care providers by the Centers for Medicare and Medicaid Services (CMS) in the United States (US). ![]()
